Riffle 1.1: pull request reviews, end to end

Riffle 1.0 could open a pull request and show you the diff, which is most of a review but not the part that finishes it. Riffle 1.1 closes the loop: leave inline comments on changed lines, build up a draft review, and submit the whole thing to GitHub without leaving the app.

Comments take Markdown, with a Write/Preview composer so you can check a code block renders before you send it, and comments come back rendered in the review drawer rather than as raw text. Review receipts confirm what actually went up, and the Review button in a review session is now somewhere you can find it instead of somewhere you have to know about.

One housekeeping note: version numbering moved to x.y.z with this release, so 1.1.0 supersedes the 1.1 that came before it.
